YOUR PROCESS
1| Schedule an appointment with our team by clicking on "Make an Appointment below.
2| Meet at the appointment center on the date and time of the appointment.
3| Get evaluated by our physician and receive your medical card within days if you qualify.
We charge $300 once you receive your medical certification. If you don't qualify you won't be charged.
4| We help you fill out the Illinois Department of Public Health Application and the state fees are as follows: $100 for 1 year card, $200 for 2 year card and $250 for 3 year card).

A: You must be able to demonstrate that you are an Illinois resident, with either a driver's license or official mail in your name sent to an Illinois address.
Q: How long does it take to get a card?
A: You will have your card within several days after your appointment.
