1. Make an Appointment
The first step to obtain a medical card from LeafyQuick is making an appointment. You can make an appointment online or call us at 1(833) 245-3239.
There are 2 options to choose from:

For clients who have not been previously diagnosed by a physician with a current qualifying condition under the Illinois Medical Cannabis Pilot Program
- Renewal Medical Card Certification 15 Minute Meeting
For clients who have been previously diagnosed by a physician with a current qualifying condition under the Illinois Medical Cannabis Pilot Program
2. See if Qualify for a Medical Cannabis Card
The second step is to prove you have all of the documentation and prerequisites in order to receive a Medical Cannabis Card in Illinois.
The following documentation is required or may be needed in the application process:
This application is to be filled by the patient and submitted along with the ‘physician written certification form’
This application is to be filled by your physician at LeafyQuick
This application is to be filled by a caregiver of a patient under 18 years of age.
This application is to be filled in by your physician at LeafyQuick if the patient is under 18 years of age
This application is to be submitted with the ‘qualifying minor patient application’ along with the ‘physician written certification form’
(Minors require a second doctor’s visit)
This application is to be filled out by the patient only if diagnosed with a terminal illness.
Additional Requirements
Additionally, to qualify for a Medical Cannabis Card, you must not:
- Hold a school bus permit
- Hold a Commercial Driver’s License
- Not be an active duty law enforcement officer, correctional officer, correctional probation officer, or firefighter
To qualify for a Medical Cannabis Card, you must:
- Be a resident of the State of Illinois at the time you apply and remain a resident during participation in the program
- Have a qualifying, debilitating medical condition
- Be at least 18 years of age or have a caregiver if under 18 years of age
You also need:
- Photograph (you can use a passport photo, or we can take a photo during your visit)
Taken in the last 30 days
Taken against a plain white or off-white background or backdrop
No filters (in natural color)
Full-face view facing directly at the camera with a neutral facial expression with both eyes open
At least 2 inches by 2 inches in size
- Proof of identity and age
Submit a clear, in color copy of your Illinois Driver’s License, Illinois State ID, or passport
- Proof of residency
Submit one additional proof of residency if your Driver’s License, Temporary Visitor Driver’s License, or State ID address matches your application.
If you submit your passport as your proof of identity, or your Driver’s License or State ID address does not match the address on your application, submit two of the following:
- Pay stub or electronic deposit receipt issued less than 60 days prior to your application date
- Voter registration card with Illinois address
- Military identification card (current)
- Bank statement (less than 90 days prior to application), or credit card statement (less than 60 days prior to application)
- Property tax bill, deed, or mortgage agreement
- Insurance policy
- Medical claim or statement benefits (dated less than 12 months prior to application)
- Tuition invoice or official university mail (dated less than 12 months prior to application)
- Utility bill (issued less than 60 days prior to application)
- Benefit Verification Letter from the Social Security Administration (DD-214)

- Neuropathy
- Tourette Syndrome
- Traumatic brain injury
- Irritable Bowel Syndrome (IBS)
- Lupus
- Hepatitis C
- Glaucoma
- Dystonia
- Multiple Sclerosis
- Rheumatoid Arthritis

3. Pay the Application Fee
With your application and documentation, you must provide a money order or check payable to the Illinois Department of Public Health. There are several options to choose from

- $200 (Two Year Registry Card)
- $250 (Three Year Registry Card)

Reduced Application Fee
- $50 (One Year Registry Card)
- $100 (Two Year Registry Card)
- $125 (Three Year Registry Card)
(The reduced fee is for those enrolled in the Federal Social Security Disability Income Supplemental Security Income (SSDI), disability programs, or Veterans. You must submit a Benefit Verification Letter that shows your name, address, and type of benefits received.)
4. Submit Your Documentation
Once you have the necessary documentation attached with your application, LeafyQuick can submit it for you to the Illinois Department of Public Health.
